Anaphylaxis Action Plan
To be completed by all families of children diagnosed with anaphylaxis. This form must be completed and signed by a doctor at the time of enrolment.
Prescribed medication and an EpiPen must also be provided.

Allergy Action Plan
To be completed by all families of children diagnosed with an allergy. This form must be completed and signed by a doctor at the time of enrolment. Any prescribed medication required as part of the child’s allergy management plan must also be provided.
